I recall seeing that same exact concept on some ancient sub drawing, by
somebody like Da Vinci. In theory I don't see why it would not work,
especially as in an ambient design it would be far easier to turn the
screw.

with a screw. On one side of the piston, ambient water, on the other

ambient air from inside sub.

You just turn the screw one way to get water in. turn the other way to
push 
water out.
