Air Service Medical, or the Air Service Medical Manual is widely quoted and listed as the earliest piece of literature on the subject... though it may or may not be of use to submariners.  It was a publication of the old War Department.

I have yet to locate a copy... but I haven't given up yet, and I have asked my rare book finder to give it a look see. 

It will probably be available, since it is so well known and widely referenced... even in modern publications.  I won't speculate on what it might cost, however.

Will keep you posted... stay tuned.

in the preface, he thanks the US Gov Printing Office for permission to reprint
from "Air Service Medical", so it was still around in '63. thanks for the offer
to help find. Often old tech articles still have great value and much
information is "rediscovered" decades later for want of access.
