I found the official Coast Guard/NTSB report on the 1973 Johnson-Sea-Link
submersible disaster in which two people died due to carbon dioxide
poisoning. It is in PDF format and located at

For those of you who have, or want to incorporate, life support in your
PSUB in the form of a carbon dioxide scrubber, this report is a *must*
read. The effect of temperature on various chemicals used as scrubber
material greatly effects its performance. Busby addresses this issue
in his book as well. The report also effectively documents the dangers
of hooks and other appendages on submersibles that can become entangled
underwater.
