[PSUBS-MAILIST] Presentation Duplication Permission Form

Hi,

After the last two conventions I spent a lot of time tracking down presentors to get them to sign a release form to allow PSUBS to copy their presentations. This year I would like to do things different. Located at:
http://www.psubs.org/convention/2004/notes/PresentationDuplicationPermissionForm.doc is a blank form.

Please would all presentors print out the form, fill it out and mail it to me before the convention. Use one form for each topic.
