Thanks Ray,

I've read some of the Archive posts on this one; will try to get to the
others soon. One post mentioned the accident might have been a collision
with a collision with a submerged log or something on that order.  Any
further information on whether it was a collision or a pressure-related
structural failure of the dome?  (To your knowledge, did the survivor ever
say?)

Also, any info on whether the dome was produced by a qualified manufacturer,
or one of the sub designer / builders own construction?
